School vaccination program
Grade 7 and 8 students
All 3 vaccines are offered to students in Grades 7 and 8 at school during school hours. Meningococcal ACYW-135 (one dose) is required to attend school. The Human Papillomavirus (HPV) vaccine (2 doses given 6 months apart), and Hepatitis B (Hep B) vaccine (2 doses given 4 to 6 months apart) are recommended.
Clinics are held 2 or 3 times during the school year (depending on the size of the school). Paper consent forms and clinic dates are sent to families through their schools. An appointment is not required to get the vaccines in school.
To receive these vaccines at school, you will need to complete a vaccine consent form. Review the consent form instructions.
Catch-up vaccine clinics
Some residents may have missed their opportunity to get the HPV, Hepatitis B, and Meningococcal vaccines at school and can still get them at Peel Public Health clinics, if eligible.
To receive these vaccinations through Peel Public Health clinics, you will need to:
- Confirm eligibility.
- Check your child's vaccination record.
- Book an appointment.
- Complete the vaccine consent form.
Meningococcal ACYW-135
- All Grade 7 to 12 students.
- Anyone born in or after 1997.
- Anyone born between 1986 and 1996 are eligible to receive the Meningococcal C vaccine.
Hepatitis B (Hep B)
All Grade 7 to 12 students.
Human Papillomavirus (HPV)
- All Grade 7 to 12 students.
- Males 9 to 26 years of age who identify as having sex with men (MSM).
Eligibility criteria applies to all doses required for the vaccine series.
Individuals who did not finish their vaccine series will only need to get the remaining dose or doses. A vaccine series does not need to be restarted even if the recommended time between doses has passed.
Eligible individuals do not need to wait between receiving the COVID–19 vaccine and any of these vaccines (HPV, Hep B and Meningococcal).
Individuals who have had COVID–19 infection may receive routine vaccinations any time after they have completed their self–isolation period and their symptoms are improving for at least 24 hours (48 hours if they have gastrointestinal symptoms such as nausea, vomiting and diarrhea).
Before booking an appointment for your child you will need to check if they’ve already received vaccines for:

- Hepatitis B (Hep B)
- Meningococcal ACYW-135
You can review your child's vaccinations in any of the following ways:
- Check their yellow vaccination card.
- View their online vaccination record.
- Call your doctor or primary care provider.
These vaccines are free if received at a Peel Public Health clinic. You will need to book an appointment. Walk–ins are not available.

After booking an appointment you will receive a confirmation email. Included in the email is a link to complete a consent form and a questionnaire.
Prepare for the appointment
- Bring your child's vaccination record (yellow card) if available.
- Give your child a snack before their appointment.
Parents and guardians should go with the student to the appointment if they require medical supervision during vaccination.

How to complete completing the paper vaccine consent form
- Fill the consent form out in pen. If a mistake is made when filling in the consent, please put a line through the mistake and put your initials beside it.
- Only the parent or legal guardian should fill out and sign the consent form. Write your full last name that you use in your health records. Do not use nicknames or shortened versions of your name.
- Choose yes or no for each vaccine (Hep B, HPV and Men-C- ACYW-135) and provide your child's vaccination history.
- Complete both sides of the consent form. A signature is required on page 2 whether choosing yes or no for the vaccines.
- You only need to provide one consent for both Hep B and HPV vaccines for all doses in the series. You should only submit a new consent if you change your mind.
If your child may not be able to answer the assessment questions being asked by the nurse at the clinic, you can print, complete, and attach to the paper consent:

Required consent
- A parent or guardian must provide consent for students younger than 14 years of age.
- Students between the ages of 14 and 16 years can provide their own consent.
- Students 16 years or older must provide their own consent.
Submitting the paper vaccine consent form
Grade 7 and 8 students
- Paper consent forms were distributed at school when students were in Grade 6 for vaccines to be given at school in Grade 7. You can print and complete a paper consent from the website if you did not receive the form.
- Return the consent form to the school by the morning of the clinic date. If the form is not received by then, we may not be able to vaccinate your child.
- Send your child's vaccination record (yellow card) with them to school for their appointment. Staple a copy of this record to the consent to avoid loss of the original vaccine record.
- All students in the class will be called down on the day of the vaccination clinic to reduce classroom disruption, but only students with consent will be vaccinated. Your child will get their vaccination by the end of the school day (or by the second clinic day in schools that have 2-day clinics).
Catch-up vaccination clinics
Required consent
- A parent or guardian must provide consent for students younger than 14 years of age.
- Students between 12 and 15 years can provide their own consent if they are considered to be capable of making their own treatment decision.
- Students 16 years or older must provide their own consent.
Submitting a vaccine consent form
After booking an appointment you will receive a confirmation email. Included in the email is a link to complete a consent form and a questionnaire. You do not have to complete a paper consent.
These vaccines are safe and those with chronic medical conditions can get them. Most people have no problems after getting vaccinated. Some may have a sore arm or redness, swelling or itching at the needle site. Occasionally tiredness, headache, or slight fever may develop.
Severe reactions are very rare and may include trouble breathing, swelling of the face or mouth, hives, seizures, or fever over 40 C.

Individuals may receive up to 3 vaccines at their appointment. Receiving these vaccines at the same time is safe.
