About COVID-19 Vaccination Services
The COVID-19 Vaccination Services is a website that allows you to enter and update certain identifying personal information and personal health information that Peel Public Health can use for the administration of the COVID-19 vaccination program. Peel Public Health is the public health unit for and a division of The Regional Municipality of Peel.
How we use your information
Your information is provided to Peel Public Health and will be accessed by Peel Public Health and provided to the provincial Ministry of Health for vaccination tracking purposes.
The information you provide through COVID-19 Vaccination Services will be used by Peel Public Health and the provincial Ministry of Health to coordinate a vaccination appointment and provide proof of vaccination as well as research, investigate, eliminate or reduce the current COVID-19 outbreak risk. Peel Public Health may also use or disclose the information as permitted or required by law in accordance with the Personal Health Information Protection Act, 2004 (PHIPA) and as set out in the Region of Peel Health Services Notice of Privacy Information Practices.
Contacting you
The information you provide in COVID-19 Vaccination Services may be used by Peel Public Health and the provincial Ministry of Health to contact you for the purposes of vaccine program administration.
Authorized collection, use and disclosure
Peel Public Health's and the provincial Ministry of Health's collection, n, use and disclosure of your information, with your permission, is authorized or permitted under the Health Protection and Promotion Act and the Personal Health Information Protection Act, 2004.
Your agreement
By inputting your personal information and personal health information into the COVID-19 Vaccination Services you are agreeing to Peel Public Health's and provincial Ministry of Health's collection, use and disclosure of your information related to scheduling and proving COVID-19 vaccination. Any other use and disclosure of this information, for example for the purpose of researching, investigating, eliminating or reducing the current COVID-19 outbreak, would only be as permitted or required by law.
More specifically, by using COVID-19 Vaccination Services, you consent to Peel Public Health and the provincial Ministry of Health collecting identifying information, including personal health information, about you that you submit through the patient verification page so that Peel Public Health and the Ministry of Health can ensure that each correctly identifies you for the purpose of administering the COVID-19 vaccination program, for scheduling purposes and for purposes of providing you proof of your vaccinations.
Peel Public Health will not further use or disclose your personal information or personal health information except for the purposes set out above or as permitted or required by law.
Entering information on behalf of someone
You must be authorized under the PHIPA to consent on behalf of someone else to enter their information into COVID-19 Vaccination Services or when giving information to Peel Public Health or the provincial Ministry of Health. For example, you are the parent or guardian of a child who is under the age of 16 years old and can consent on behalf of the child.
Proper use of COVID-19 Vaccination Services
Do not enter information into COVID-19 Vaccination Services that may infringe the rights of any other person or entity.
You must not:
- Impersonate or make any misrepresentations of any other person or entity.
- Submit information that you know is false or inaccurate.
- Use COVID-19 Vaccination Services to gain entry or access to information you are not lawfully permitted to access, view or use, or to gain unauthorized entry to the computer networks or systems used by COVID-19 Vaccination Services.
- Interfere with or disrupt any of the computer networks or systems used in connection with COVID-19 Vaccination Services.
- Violate any laws or regulations applicable to the protection of personal information or personal health information.
- Violate any generally accepted internet practices or interfere with any other person's ability to use COVID-19 Vaccination Services.
- Reverse engineer, decompile, modify or attempt to discover the COVID-19 Vaccination Services source code or otherwise reduce COVID-19 Vaccination Services to a human-perceivable form.
- Circumvent any technology used by COVID-19 Vaccination Services to protect the COVID-19 Vaccination Services and associated databases.
Terminating your use
Peel Public Health may terminate or refuse to let you use COVID-19 Vaccination Services at any time. This can be for any reason and without notice.
You agree that even if Peel Public Health does not insist on or enforce an item in this agreement, they still have the right to insist on and enforce that item
Security
You are responsible for the security of the device, browser, and network that you are using to access COVID-19 Vaccination Services. You are also responsible for the security and privacy of your email account, mobile phone number and alternate phone number if you provide your email address, mobile phone number or alternate phone number using COVID-19 Vaccination Services and consent to receive email, text, or voice communication.
Peel Public Health's security practices are continually evolving to ensure that the integrity and confidentiality of information and systems are maintained. Peel Public Health uses up-to-date security safeguards to protect its networks and websites from misuse, alteration, copying, disclosure, destruction, monitoring or unauthorized sharing of information and damage. The government's safeguards include the use of security software and encryption protocols and involve physical, technical, and procedural controls to protect information behind the Peel Public Health firewall.
Audit
Your access to and use of COVID-19 Vaccination Services may be recorded and audited by Peel Public Health to respond to an inquiry from you, and to investigate any privacy breach incidents as required.
Disclaimer
Peel Public Health is providing COVID-19 Vaccination Services on an "as is", "as available" basis. Peel Public Health does not make any express or implied warranties, representations or promises with respect to COVID-19 Vaccination Services including, but not limited to warranties as to usefulness, completeness, accuracy, reliability, or lack of computer virus.
No liability
Peel Public Health is not responsible, and will not be liable to you or anyone else, for your ability or inability to use COVID-19 Vaccination Services. This is even if Peel Public Health has been advised of the possibility of damages.
This includes any:
- losses
- expenses
- costs
- damages
- liabilities
- causes of action, actions, claims, demands, lawsuits or other proceedings
