    About us

    About Adult Day Services and our team.

    We believe that everyone plays an important role in the client’s journey. Clients, caregivers, and families can add great value to the care team.

    Our focus is to improve the health care experience and outcomes for clients, caregivers, and families. We work with clients, caregivers, and families to identify and achieve their goals.

    Goals include:

    • Helping clients improve or maintain their best possible physical and cognitive functionality.
    • Promoting personal dignity and self-esteem by encouraging independence.
    • Providing a safe, secure, and stimulating environment for care.
    • Supporting clients, their caregivers, and families to be involved in their care and decisions.

    Connecting Care Act, 2019

    Refer to the Ontario government’s patient bill of rights for details about how every health service provider and Ontario Health Team needs to ensure clients’ rights are fully respected and promoted.

    Managers

    The Community Support Services Manager provides leadership and direction to manage the community support services programs and their delivery across multiple sites.

    These programs include Adult Day Services, across 4 sites. They are Davis Centre in Bolton, Tall Pines in Brampton, and Malton Village and Sheridan Villa in Mississauga. Social work support for clients and caregivers is also included. The manager is also responsible for overseeing our virtual adult day services which is available at no cost.

    The Integrated Care Manager provides leadership and direction to community support services programs located at the Seniors Health and Wellness Village at Peel Manor. These programs include Adult Day Services, short stay overnight respite beds and the Integrated Care Centre which provides primary care and social work services.     In their roles, both managers collaborate with a variety of community health service providers and at the provincial level. This includes Ontario Health and Home and Community Care Support Services. These partnerships focus on the planning and coordination of e-services that will meet the needs of older adults in Peel region.

    Supervisors

    The supervisor manages Adult Day Services and provides leadership to the team ensuring that clients receive coordinated, collaborative, quality care and services. The supervisor also oversees the inter-professional team within the program and liaises with external partners.

    Support worker

    Adult Day Services support workers provide personal care to clients during their time at the program. This includes assisting clients with activities of daily living such as using the washroom and bathing; supporting them during restorative programs with eating, and health and wellness activities; as well as managing any medications they need to take while attending the program.

    Adult Day Services assistant

    Adult Day Services assistants provide therapeutic and recreational services to support the independence of clients. They focus on providing programs to support clients’ physical, social, recreational, intellectual, emotional, and spiritual needs. They are also responsible for planning and delivering community outings and special events.

    Coordinators

    The coordinator works with clients and caregivers who are on the waitlist to coordinate program admission. They also work with registered clients, their caregivers, and other health providers by handling case management to support client’s plan of care. They maintain a current knowledge of community resources and available education sessions and make appropriate referrals based on client and caregiver’s individual needs.

    Registered nurse

    The Registered Nurse assesses the needs of clients and works with the rest of the team to support the client or their caregiver to attain or maintain the client’s optimal function.

    Social worker

    The social worker is available on a referral basis for short term support to help clients and families experiencing change, loss, or grief, as well as caregivers in need of transition-navigation support across the health care system.
