- No use of biological indicators;
- Inability to monitor that physical parameters of the reprocessing cycle have been achieved (pressure);
- Inadequate staff training on reprocessing, documentation, and recall process.

- Implement routine use of biological indicators.
- Implement a system to record physical parameters of the reprocessing cycle.
