    How the IPAC lapse was identified: 
    During a routine compliance inspection.

    Summary description of the IPAC lapse

    • Observed expired needles for micro-channeling, micro-needling, micro-blading, and Organic Permanent make-up.
    • Observed opened packages of needles for micro-channeling, micro-needling, micro-blading, and Organic Permanent make-up with clients’ names.
    • Observed a basket full of used needles for micro-channeling, micro-needling, micro-blading, and Organic Permanent make-up (OPM) with clients’ names.

    IPAC lapse investigation

    Did the IPAC lapse involve a member of a regulatory college? No.

    Were any corrective measures recommended? Yes.

    Details of the corrective measures

    Issued a Section 13 order the Health Protection and Promotion Act to immediately:

    • Stop providing micro channeling services.
    • Stop reusing single use sharps such as the needles used for micro-channeling, micro-needling, micro-blading, and Organic Permanent make-up and any other invasive services.
    • Dispose of all damaged, opened, and used packages of needles, and sharps in the sharps container immediately after 1 use.
    • Dispose of all expired sharps such as the needles used for micro-channeling, micro-needling, micro-blading, and Organic Permanent make-up and any other invasive services.
    • Provide client records for all clients who received invasive facial services micro-channeling, micro-needling, micro-blading, and Organic Permanent make-up as of May 2022 to Aug 2022 forthwith to Peel Public Health inspector.

    • 2022-08-26: Verbal order issued under section 13 of the Health Protection and Promotion Act.
    • 2022-08-26: Conditional Pass inspection summary sign was issued and posted at the entrance to the facility.
    • 2022-08-26: Stop Service Red summary sign was also issued and posted at the entrance to the facility.
    • 2022-08-30: written order was issued under section 13 of the Health Protection and Promotion Act.
    • Charges pending.
    • Further action and follow-up pending.

    • Verbal order was issued on 2022-08-26 under section 13 of the Heath Protection and Promotion Act.
    • Written order was issued on 2022-08-30 under section 13 of the Health Protection and Promotion Act.
    • Charge (Offence Notice - Part 1) was issued on 2022-09-17 under Ontario Regulation 136/18 (Personal Service Settings Regulation).

    • 2022-09-02 re-inspection was conducted and an adequate supply of single use sterile needles was available on-site for the micro-channeling service. This service was permitted to resume.
    • 2022-09-02 reinspection confirmed that sharps are being disposed of immediately after use in a sharps container.
    • 2022-09-07 client records for invasive facial services were received and reviewed. There was no evidence of sharps being used more than once. Communication to clients was not necessary. No further action required.
