Self reported IPAC lapse in reprocessing of dental instruments including:
- Reprocessing failure - critical and semi-critical dental instruments were not sterilized and subsequently put into circulation.

Details of the corrective measures
- Sterilize the affected instruments.
- Ensure policies and procedures on reprocessing are consistent with the best practice guidelines.
- Conduct staff education on reprocessing, including documentation.
- Increase auditing and supervision of newly trained staff.

Additional comments:
- The lapse was resolved by the clinic staff before Peel Public Health was notified.
- An on-site investigation was conducted; recommendations for improving staff training and supervision, as well as redundant checks were provided to the premise owner.
- Dental clinic will proceed with patient notification.
