To order vaccines under high risk programs, fill out the appropriate requisition form and fax to the number indicated on the form:
- Hepatitis A vaccine requisition
- Hepatitis B vaccine requisition – for contacts of Hepatitis B case/carrier (including neonates) and Hepatitis C cases
- Meningococcal vaccine requisition
- Human Papillomavirus (HPV) vaccine requisition for MSM
To order High Risk Hepatitis B vaccine, download the High Risk Hepatitis B vaccine requisition, complete and click submit button. Faxed forms will no longer be accepted.

Paid delivery
Enroll in the paid-delivery program service provided by Critical Path Ltd. To enroll in the vaccine paid-delivery program use the Critical Path Account Activation Form.
