    Help reporting your child’s vaccinations online

    Step-by-step instructions on how to report or view your child’s vaccinations.

    Before you start, make sure you have your child’s:

    • Ontario Health Card (OHIP)
    • Vaccination record

    If you don’t have these, call Peel Public Health at 905-799-7700 or 1-888-919-7800 and ask for Immunization Records. They’ll help you get online access, or you can report immunizations over the phone.

    If you’ve already set up an account for your child but they recently turned 16 years or older, they must call Peel Public Health to unlock their account. The account cannot be unlocked without the child’s consent.

    Getting started

    • Go to the child vaccination reporting page and select “View or report vaccinations”.
    • Select “Get Started”.
    • Choose the type of device you’re using, “Personal” for your own phone or computer, or “Public” if it’s a shared device like a library computer.
    • Read the Acceptable Use Policy and select “I accept” to continue.
    • If you don’t accept, you can report by phone. Call Peel Public Health and ask for Immunization Records.

    Reporting vaccinations for the first time

    To report for the first time, log in using either your child’s Ontario Health Card number or their Ontario Immunization ID and Ontario Health Card number. If you received a letter from Peel Public Health, you’ll find a 10-digit Ontario Immunization ID number in the top of the letter.

    If your child doesn’t have an Ontario Health Card, call Peel Public Health and they’ll set up an account that you can use without one.

    Using an Ontario Health Card

    Complete the fields on the form:

    • Health card number
    • Version code
    • Stock Control Number from the back of the card
    • Child’s name, sex, and date of birth
    • Postal code

    Choose who the card belongs to - select “A Dependent” if you are submitting vaccination information for your child.  Only select “Me” if you are submitting your own immunization information. Then select “Verify Patient”.

    Using an Ontario Immunization ID

    • Enter the number and select “Verify ID”.
    • Choose who the card belongs to - select “A Dependent” if you are submitting vaccination information for your child.  Only select “Me” if you are submitting your own immunization information.
    • Enter the Ontario Health Card number. Then select “Verify Patient”.
    • Create a 6-digit PIN and confirm it. Enter your email and select “Set PIN”.
    • You can use the same PIN if you have more than one child.
    • After the PIN is set, select “OK”. You’ll now be able to view your child’s vaccination record.
    • After you’ve logged in and set your PIN, select “Submit Immunizations”.
    • Answer the questions:
      • Did you receive a letter from Peel Public Health?
      • Were the vaccines given in Ontario?
    • Then, select “Add a Date & Immunization” to begin entering vaccines.
    • Enter each vaccine by name and date received. Start typing the vaccine name, then choose it from the drop-down list and select “Save”. If you’re not sure what to enter, use the schedule on the right side of the screen for guidance.
    • Once you’ve entered all vaccines, select ‘Save and Proceed to Documents’.
    • Upload a clear photo of your child’s immunization record (yellow card). We encourage you to upload a copy of your record so we can make sure everything is correct when we review the submission.
    • Choose the file and select “Save and Proceed to Patient”.
    • Review and confirm your child’s information. Then enter your own information as the person submitting the immunization record.
    • Select “Save and Proceed to Review”. Make sure all details are correct, then select “Submit Immunizations”.
    • You’ve successfully reported your child’s vaccinations to Peel Public Health! You’ll receive a submission reference number that you can track.
    • You may need this number when registering your child for school.
    • You will also receive an automatic email from the online reporting system when Immunization Records staff have reviewed your submission.  When you get this email, you can check online if any immunizations are still needed.
    • Every time your child gets a vaccination, use this process to report it to Peel Public Health.
