Summary description of the IPAC lapse
Self reported IPAC lapse in reprocessing of dental instruments including:
Reprocessing failure - critical and semi-critical dental instruments were cleaned but not sterilized and subsequently put into circulation.

Details of the corrective measures
- Sterilize the affected instruments.
- Ensure policies and procedures on reprocessing are consistent with the best practice guidelines.
- Increase auditing and improve consistency in the use of sterilization and cleaning logs.
- Conduct staff education on reprocessing; including monitoring of physical, biological, and chemical indicators.
Additional comments:
- The lapse was resolved by the clinic staff before Peel Public Health was notified.
- 1 of 2 non-sterile instrument packages were removed from circulation prior to use and immediately sterilized.
- An on-site investigation was conducted; recommendations for improving quality and consistency were provided to the premise owner.
- Dental clinic will proceed with patient notification.
