Summary description of the IPAC lapse
Deficiencies in reprocessing of dental equipment, including:
- Reprocessing and subsequent re-use of single use items.
- Not keeping items sterile to the point of use.
- Inability to monitor that physical parameters of the reprocessing cycle have been achieved (no recording device).
- Inadequate staff training on reprocessing, documentation, and recall process.

Details of the corrective measures
- Dispose of used dental trays following use as per manufacturer’s instructions.
- Dispose of used endodontic files; use in accordance with manufacturer’s instructions.
- Re-sterilize observed unpackaged burs and keep sterile to the point of use.
- Follow product monographs and manufacturer’s instructions for use for dental equipment and reprocessing devices.
- Conduct formal staff education on reprocessing including monitoring and recording biological, chemical, and physical parameters and the recall process.
