- Reprocessing failure - semi-critical equipment was not sterilized and subsequently put into circulation

Details of the corrective measures
- Clean and high level disinfect semi-critical equipment between each patient use
- Ensure policies and procedures on reprocessing are consistent with best practices
- Conduct staff education on reprocessing
- Increase auditing of the reprocessing process
