Tuberculosis: for health professionals
Ordering vaccines and medication, reporting cases, clinic referrals, diagnostic tests, and TB in long term care and retirement homes.
Follow the instructions on the appropriate form to submit your order request or reporting notification to Peel Public Health:
- Report latent TB infection (LTBI) and order LTBI treatment using the reporting positive skin test or IGRA and TB drug order form
- Physician report form: medical surveillance for tuberculosis
- Report a TB case using the notification of new active or reactivated tuberculosis case form
Referral forms
Refer patients to the William Osler Health System TB clinic using the tuberculosis clinic referral form - You will need to download the file and open it in Acrobat Reader.

Diagnostic tests
There are two accepted tests for identification of latent tuberculosis infection (LTBI), including tuberculin skin test and blood test (interferon gamma release assay, or IGRA).
