Peel Public Health offers support to parents and caregivers at each stage of parenting. All services are free of charge and translation is available.

Resources
Healthy Babies Healthy Children screening form
This screening form is used to refer pregnant people, and families with infants and young children to this free program. Peel Public Health nurses provide in-person home visits.
Breastfeeding services referral form
This form is used to refer clients in need of breastfeeding support. Peel Public Health nurses will reach out to assess their specific needs. Fax the completed form to 905-791-8981.

To access the referral form, go to find a doctor or service and follow these steps in the ‘find a service/referral info’ section:
- Type ‘mental health’ in the ‘Search by the clinic or program name’ box.
- Select ‘Adult and Paediatric Outpatient Mental Health Intake and Triage – All Sites’
- Access the PDF referral form by selecting ‘Download Form’ found under the ‘Referral information’ heading.
