If you live in Mississauga, Brampton or Caledon and have been placed on Medical Surveillance by Immigration, Refugees and Citizenship Canada, follow these steps:
Call Peel Public Health at 905-799-7700
If you have already talked to Peel Public Health about medical surveillance, follow the instructions we provided to you.
If you haven't called Peel Public Health:
- You will need to have your form with you.
- We will ask you some questions about your form.
After talking to Peel Public Health
Print the Letter for your Doctor and Doctor Report Form and take to your doctor.
