If you live in Mississauga, Brampton or Caledon and have been placed on Medical Surveillance by Immigration, Refugees and Citizenship Canada, follow these steps:
If you have already talked to Peel Public Health about medical surveillance, follow the instructions we provided to you.
If you haven't called Peel Public Health:
Print the Letter for your Doctor and Doctor Report Form and take to your doctor.
If you have questions about why you were placed on medical surveillance, you can email Immigration, Refugees and Citizenship Canada.
