Request an ambulance call report
Overview
Paramedics complete an ambulance call report each time youre under their care. This report has information such as your name, address, and medical history.
It also states why you called 911, what treatment you received, and other details of their time with you.
This service is for any Peel resident or person working in or visiting Peel who needs a report of an ambulance incident.

Eligibility
Any Peel resident or person working in or visiting Peel is eligible for this service.
What you need
You will need the following information about the patient to complete the request:
- Name of patient as on Health Card or used for documentation.
- Date of birth
- Home address
- Location ambulance was called to
- Date and time of incident
- If transported, name of hospital
- Contact information that paramedics staff can contact you
Fees
There is a $75 fee for this service. You can pay this fee by certified cheque or credit card.
Payment can be made by mail or online.
