School dental screening results

The Peel Public Health School Dental Screening Results form was previously called Peel Public Health Dental Screening Report Card.

The form will be sent home with children that receive a dental screening at elementary school. The form lets you know the results of your child’s school dental screening and if they need dental care. If your child needs dental care, you should contact your dental provider.

If you’re not able to afford dental care for your child, they may be eligible for free dental services through Healthy Smiles Ontario.

If the registered dental hygienist did not see any visible dental issues in your child’s mouth, continue to take your child for regular dental visits. A dental screening does not replace regular dental visits.

This information is being collected pursuant to the Health Protection and Promotion Act R.S.O. 1990 c. H. 7 and will be retained, used, disclosed and disposed of in accordance with all applicable municipal, federal and provincial laws and regulations governing the collection, retention, use, disclosure and disposal of information including the Municipal Freedom of Information and Protection of Privacy Act R.S.O. 1990 c. M. 56 and the Personal Health Information Protection Act 2004 S.O. 2004, c. 3. This information will be used to deliver oral health programs and services to children within Peel and may be used for program planning and evaluation purposes.
