This tool is meant to derive the capacity level of agencies in terms of sustainability. Please rate how well your organization performs or accomplishes the indicator statements.

Five dimensions identified for this assessment include:

  1. Community Impact
  2. Financial Sustainability
  3. Sectoral Engagement/Partnership
  4. Leadership
  5. Program/Service Delivery Capacity

The tool includes 23 statements on a 5 point scale, on a continuum from not meeting the performance requirement, to exceeding it.

The scale is defined as follows:

Section 1: Community Impact
