Withdrawn Regional Chair Candidates

Under Bill 5, all previously registered Regional Chair candidates are deemed to be withdrawn as of July 27, 2018, immediately before 2 p.m.

All previously registered candidates are required to complete and file this form with Regional Clerk by March 29, 2019, 2 p.m.:

• Financial Statement and Auditor’s Report (Form 4)

The following forms can also be completed, if needed:

• Financial Statement – Subsequent Expenses (Form 5)
• Notice of Extension of Campaign Period (Form 6)

The original form must be filed. Photocopies or scanned documents will not be accepted.

To receive the $200 nomination fee refund, the candidates must file Form 4.
