Freedom of information | Formal Requests

To request formal access to a record, access to your personal information, or a correction to your personal information, write a letter or forward a completed Access Request form (PDF) to:

Attention: Regional Clerk and Director of Clerks

Request should include:

  • mandatory, non-refundable $5.00 application fee
    (Cheque or money order, made payable to the Region of Peel)
  • the type(s) of record(s) you are requesting
  • specify names or types of records, including dates where possible
  • authorization and direction form, if the request is submitted by an agent for the requestor. This form must be addressed to the Region of Peel.

Providing as much detail as possible helps staff conduct a faster search.

Request Process

Upon receipt by the Regional Clerk's office, your request is date stamped and forwarded to the Manager, Regulatory Compliance for processing.

An acknowledgement letter will be forwarded to you within 3 business days. The legislation stipulates that an institution must provide the requester with the information and/or a decision regarding their request within 30 calendar days from the date a complete request is received.

On occasion, an institution may ask for an extension, if the request is for an extensive number of records, and/or if an outside consultant needs to be contacted.
